Please review the COVID 19 Self-Assessment Questionnaire prior to attending any prep skates or Tryouts.

Are you currently experiencing any of the
following symptoms?
Symptoms should not be chronic or not related to other
known causes or conditions.
(Fever/Chills, Cough/Barking Cough (croup), Shortness
of Breath, Sore Throat, Difficulty Swallowing, Loss of Smell/Taste, Pink Eye,
Runny/ Stuffy/Congested Nose, Headache, Nausea/Vomiting, Diarrhea, Stomach
Pain, Muscle Aches, Extreme Tiredness, Falling Down)
· Has a Doctor, Healthcare Provider, or Public Health Unit told you that you should currently be isolating (staying at home)?
· In the last 14 days, have you been identified as “close Contact” of someone who currently has Covid-19?
· In the last 14 days, have you received a Covid Alert Exposure notification on your cell phone?
(If you already went for a test and got a negative result you are a “No”)
· In the last 14 days, have you or anyone you live with traveled outside of Canada?
· Is anyone you live with currently experiencing any new Covid-19 symptoms and/or waiting for test results after experiencing symptoms?
If you answer YES to any of these questions stay home and you will not be permitted entrance to the facilities.
