Covid – 19 Self-Assessment Questionnaire

Please review the COVID 19 Self-Assessment Questionnaire prior to attending any prep skates or Tryouts. 

·        Are you currently experiencing any of the following symptoms? Symptoms should not be chronic or not related to other known causes or conditions. (Fever/Chills, Cough/Barking Cough (croup), Shortness of Breath, Sore Throat, Difficulty Swallowing, Loss of Smell/Taste, Pink Eye, Runny/ Stuffy/Congested Nose, Headache, Nausea/Vomiting, Diarrhea, Stomach Pain, Muscle Aches, Extreme Tiredness, Falling Down)

 ·        Has a Doctor, Healthcare Provider, or Public Health Unit told you that you should currently be isolating (staying at home)?

 ·        In the last 14 days, have you been identified as “close Contact” of someone who currently has Covid-19?

 ·        In the last 14 days, have you received a Covid Alert Exposure notification on your cell phone?

          (If you already went for a test and got a negative result you are a “No”)

 ·        In the last 14 days, have you or anyone you live with traveled outside of Canada?

 ·        Is anyone you live with currently experiencing any new Covid-19 symptoms and/or waiting for test results after experiencing symptoms?

 

If you answer YES to any of these questions stay home and you will not be permitted entrance to the facilities.
